Overview

This course provides the skills and knowledge to conduct an effective incident investigation in the workplace. The course outlines the need for investigation of incidents, NSCA Causes of Accidents Model, categories of hazards, hierarchy of risk controls, cause and effect analysis and fault trees analysis. 

Who should attend?

Personnel with roles and responsibilities in the area of accident investigation including supervisors/frontline managers, managers, OHS personnel, engineering and maintenance staff and OHS committee members.

Learning Outcomes

Upon completion of this course, participants will be able to:

• Discuss the personal, business and legal reasons for investigating incidents

• Describe, using the NSCA model for causes of incidents, the relationship between hazards and breakdown in the organisation’s OHS management systems

• Identify, with reference to categories of hazards, the contributory factors which are the direct cause of the incident and their means of control

• Conduct an investigation of a given incident using the incident Investigation Worksheet

• Use the appropriate analysis methods to determine contributory hazards, management system breakdowns and required improvements for a given accident.

Award

No formal assessments will be undertaken for this course. Students attending this course will be given a Statement of Attendance.
